Join DaVita Integrated Kidney Care (IKC) as a Care Coordinator – Nurse Practitioner Primary Care Pilot
The Opportunity
It takes a village to care for our patients, and as a Care Coordinator (CC), you’ll be at the forefront of healthcare innovation. This is not your standard scheduling role; you will be a key player in a specialized program where Nurse Practitioners (NPs) serve as independent, "primary care-lite" providers for complex CKD and ESKD patients.

Location & Logistics
Work from your home office with quarterly travel for team-building and nephrology office visits in Des Moines, IA and Lincoln, NE.
Zone & Timing: Must be based in or aligned with Central Time (CST) to support our NE and IA markets.
Schedule: Full-Time, Monday–Friday. Daily start time between 7:00–8:00 AM
Home Office Requirements: Quiet, private space with high-speed internet.
Responsibilities
• Strategic Scheduling: Own the scheduling process for NP patient panels. You will balance complex schedules, facilitate outreach to patients for upcoming visits, and ensure the NP’s time is optimized for high-impact patient interactions.
• Care Coordination: Assist in navigating patients through the continuum of care. Help move patients through identified care pathways under the guidance of the NP and the local nephrology practice team.
• Communication Hub: Act as the primary liaison between patients, the NP, and the partner nephrology office. Manage inbound and outbound communications to resolve scheduling conflicts, confirm appointments, and collect patient information.
• Innovation Partner: Participate in workflow optimization. Refine processes and improve patient experience.
• Documentation & Compliance: Maintain accurate, real-time documentation in the Electronic Medical Record (EMR) to ensure care teams have the information they need to succeed.
• Resource Navigation: Identify and coordinate resources, referrals, and appointments, collaborating with the broader care team to ensure patients have access to the care they need.
Requirements
• High School Diploma or GED required.
• Experience: 2+ years of experience in a professional office or clinical environment.
• Tech-Savvy: Proficiency in Microsoft Office (Excel, Outlook) and experience with Electronic Medical Records (EMR).

Nice-to-haves
• Passion for the Mission: A genuine interest in nephrology, dialysis, or primary care. You are excited about the challenge of improving health outcomes for CKD and ESKD patients.
• Clinical Office Experience: 1–3 years of experience in a medical office, particularly in scheduling, referrals, or care management.
• Clinical Exposure: CNA, MA, or similar clinical background is a major plus.
• One year of experience using Cerner (EMR)
