<div>Licensed Vocational Nurse (LVN) – Home Health<br><br>Citie's where we are looking to have LVN coverage : Los Angeles, Hesperia / High desert</div><div><br></div><div>Our agency’s mission is to provide professional and paraprofessional services to clients in their</div><div>homes assisting them to achieve the highest level of potential in their day-to-day self-care</div><div>activities.</div><div><br></div><div>Job Description:</div><div>- Provide direct patient care in a home setting, ensuring the comfort and well-being of</div><div>patients</div><div>- Administer medications and treatments as prescribed by the physician (Wound Vac, IV</div><div>Certified, Wound care, etc.)</div><div>- Monitor and record vital signs, such as blood pressure, temperature, and heart rate</div><div>- Implements plan of care initiated by the registered nurse / Referring doctor</div><div>- Educate patients and their families on proper healthcare procedures and disease</div><div>management</div><div>- Provides accurate and timely documentation consistent with the plan of care</div><div>- Participates in coordination of home health services, appropriately reporting the</div><div>identified needs for other disciplines (HHA, OT, PT, MSW, ST,) to the registered nurse</div><div>and/or Clinical Supervisor/Nursing Supervisor</div><div>- Participates in personal and professional growth and development.</div><div>-Uses equipment and supplies effectively and efficiently.</div><div>The above statements are intended to be a representative summary of the major duties</div><div>and responsibilities performed by incumbents of this job.
