OVERVIEW OF MESA COUNTY PUBLIC HEALTHThe following duty statements are illustrative of the essential functions of the job and do not include other non-essential or marginal duties that may be required.
Provide home visits to women and their families to assess physical, emotional, social, and environmental needs as they relate to health and life course development including establishing realistic goals and measurable outcomes for promoting their development.
Provide education and instruction to assist women and their families in attaining targeted goals in areas including prenatal and postpartum care, nutrition, parenting, well child care, family planning, and special health problems; evaluates women and families’ progress toward targeted goals; revises plan of care as appropriate.
Link women and families with community resources relevant to their specific needs that may include basic needs such as food, shelter, and transportation.
Collaborate care plans with other care providers as indicated by the program.
Demonstrate time management by flexibly scheduling and rescheduling a day’s activities based on multiple factors, including client need.
Record nursing activities promptly and according to program protocols.
Consistently maintain a caseload of 25 active clients.
Utilize data routinely to understand and improve job performance.
Build and maintain relationships with a diverse range of clients over a period of time.
Work to understand and address personal reactions to clients, welcome feedback non-defensively, and make good use of the reflective supervision provided.
Work collaboratively with coworkers, other departments and leadership to promote program and agency objectives and improve client services.
Actively participate in and promote programs within community coalitions, advisory boards, councils and MCPH events.
Act as a public health role model and exemplify the principles of social justice, increase cultural competence, and work to close gaps in health outcomes that impact populations disproportionately.
Perform community outreach to promote program referrals and keep relevant knowledge of client resources.
Participate in program orientation of students and new NFP nurses including mentoring and providing consultation.
Demonstrate proficiency in Microsoft Office, Google Suite and electronically generated nursing documentation.
NON ESSENTIAL JOB FUNCTIONS
Participates in emergency planning and response activities.
Participates in agency-wide activities that may include strategic planning, quality improvement, and/or staff development, as appropriate.
Completes necessary training through FEMA, including Incident Command Structure (ICS) courses.
When a local declaration of emergency or disaster is declared by County leadership, employees may be required to work as part of a National Incident Management System (NIMS).
Performs other duties as assigned, including support of public health measures benefiting the community at large.
MINIMUM QUALIFICATIONS REQUIRED
Education and Experience
Bachelor’s Degree in Nursing (BSN)
Experience in maternal/ pediatric health, public/ community health, behavioral health, labor/delivery nursing or any other nursing specialty to meet the requirements of the job, preferred
Fluency in Spanish and English, preferred
Certificates and Licenses
Active Colorado Registered Nurse license in good standing
Current CPR certification
A valid Colorado driver’s license with a responsible driving record with access to a personal vehicle that can be used during work hours for nurse transportation
Computer and Technology
Knowledge and ability to use Microsoft Office applications including, Word, Excel, and PowerPoint.
Experience with Google Applications including Gmail and Drive.
PHYSICAL REQUIREMENTS and WORKING ENVIRONMENT
Able to complete duties under stress, deadlines, and competing priorities
Visual/hearing ability sufficient to comprehend written/verbal/non-verbal communication
Ability to perform tasks involving physical activity, which may include lifting of supplies, equipment, or babies and small children; extensive bending, standing, walking, or sitting
Ability to work from home in a private and secure area and hold visits in various settings, including client homes, outdoors, and in public places
Ability to travel throughout designated area to multiple locations, including to Denver for periodic trainings and meetings
May be exposed to communicable diseases and infections
