</div><div>We are looking for a licensed Physical Therapist (PT) to provide home health visits to patients in their homes. The PT will evaluate patients, develop care plans, and provide therapy to improve mobility and function.<br><br>

<li> Perform patient evaluations </li>
<li> Provide physical therapy treatments </li>
<li> Develop and follow care plans </li>
<li> Educate patients and caregivers </li>
<li> Complete documentation on time </li>

<li> Active California PT License </li>
<li> Home health experience preferred </li>
<li> Valid driver’s license and reliable transportation </li>
<li> CPR certification</li>
